CNIB has developed a survey for diabetes educators, public health nurses and community health workers to help identify possible gaps in knowledge around diabetic retinopathy, how to best meet the needs of people living with diabetic retinopathy and how to best support diabetes educators in working with this group. The information gathered from the survey will help guide CNIB in creating print and online resources for Canadians with diabetes/diabetic retinopathy, their caregivers and diabetes educators.

We ask that you to complete the survey below, English and French versions available, to your colleagues or members. The deadline to complete and submit the survey is Monday, January 30, 2012.
